2. Along with the completed application form, present the following documents to the COK representative:
a. Valid Identification (Drivers License, Voters ID, Passport)
b. Proof of address ( Last two months’ utility bill)
c. Tax Registration Number (TRN) Card/Proof of TRN
d. ATM Plus Access Card (if you don’t have a card, you may apply for a card during the process)
3. Your COK representative will complete the process and assign you your Password and PIN.
